Abstract

Despite extensive research on behavioral and environmental determinants of dental caries, the role of host genetic factors such as the ABO blood group system remains inadequately characterized, leaving a critical gap in individualized caries risk assessment. Therefore, it is of interest to evaluate the association between ABO blood groups and dental caries susceptibility, while exploring the underlying immunological, microbial and genetic mechanisms. A consistent trend emerges from the literature showing higher caries burden among individuals with blood groups B and AB, attributed to reduced salivary IgA levels, enhanced bacterial adhesion mediated by non-O blood group antigens and differences in secretor status. Individuals with blood groups O and A appear comparatively less susceptible, possibly due to superior mucosal immune defense and less favorable antigen-mediated microbial binding sites. Thus, data shows the ABO blood group antigens to caries pathogenesis, establishing a rationale for incorporating genetic profiling into personalized preventive dentistry protocols.
