Abstract

Schizophrenia is a chronic psychiatric disorder associated with substantial disability, persistent symptom burden and premature mortality. Therefore, it is of interest to compare the 12-week effectiveness and safety of olanzapine and risperidone monotherapy in 40 newly diagnosed patients with schizophrenia-spectrum disorders in a rural Indian tertiary-care setting. Both groups showed significant clinical improvement, but olanzapine showed greater reduction in PANSS positive, negative and total scores by month three, with total PANSS scores of 62.1 ± 8.7 versus 69.3 ± 9.4 in the risperidone group. Olanzapine was associated with greater mean weight gain and more weight-related adverse events, whereas risperidone was associated with a higher frequency of extrapyramidal symptoms. Thus, data shows the importance of antipsychotic selection based on expected efficacy, metabolic risk, and neurological tolerability and structured follow-up monitoring.
